If you are here it means your name was on the list of accepted band members for the CMEA/CBDA All-State High School Honor Band. If you have not already checked the lists, please go back and do so now.

Next you will need to download all of the appropriate forms. Click on the links below to download the forms. Make sure to return all forms with deadlines to the appropriate address as listed on the form before the January 15, 2010 deadline with your $325 participation fee.

ANYONE WHO FAILS TO RETURN THEIR FORMS AND FEES BY THE DEADLINE WILL BE REPLACED.
